ProHealth Home Care one of Wisconsin's most comprehensive sources of medical care and hospice care provided in the home. We provide a seamless continuum of care for people of all ages. Residential hospice care is offered at ProHealth AngelsGrace Hospice, a beautiful, 15-bed hospice nestled on eight scenic acres in Oconomowoc. ProHealth Care also offers quality in-home hospice care and in-home skilled nursing care. We are state-licensed, Medicare-certified and accredited by the Joint Commission.
Our highly skilled nurses provide a wide array of services, including medication education and management, injections and intravenous therapy, wound care, nutritional counseling, bowel and bladder management and much more. Physical, occupational and speech therapists address home safety concerns, strengthening and rehabilitation needs. Our licensed social workers offer assistance with community resources, financial assistance and supportive care and counseling to patients and families.
Hours: Weekday availability 2-3 days per week as needed from 8-430pm.

The Physical Therapist in ProHealth Care’s Home Health team works collaboratively with patients, nurses, social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care environment. Assesses plans, facilitates and evaluates timely coordination of quality, cost-effective care and works flexible hours and has 24-hour accountability as necessary to meet patient/family care needs.
Responsibilities / Accountabilities:
Collects client health data, prioritized by the client’s condition or needs, utilizing appropriate techniques while involving the client, family, and other health care providers, as indicated.
Ensures systematic, on-going, data collection with relevant documentation.
Analyzes the assessment data to identify immediate and long-term client care needs.
Identifies individualized expected outcomes in partnership with the client, family and other health care providers.
Develops a plan of care, in consultation with patients and caregivers, based upon identified problems, prescribes interventions to attain expected outcomes.
Supervise support staff in the delivery of PT services, including physical therapist assistants and home health aides, following regulatory and professional standards.
Responsible for the client’s appropriate and uninterrupted care throughout the health care continuum, by coordinating services, discharges planning, and community resources.
Demonstrates competence in the clinical knowledge, skills, and technology utilized in client care, based on department-specific and organizational standards.
Continually evaluates the client’s progress toward achievement of outcomes.
Bachelors in Physical Therapy required.
Physical Therapist - WI License
Valid Wisconsin Driver's License.
At least 6 months in a home health setting preferred
