VCH Care Coordination:
The Monroe Carell, Jr Children's Hospital at Vanderbilt Department of Care Coordination is a collaborative of highly trained health care professionals that deliberately organizes patient care activities that bridges gaps along the care pathway for children. The case managers are ultimately responsible for meeting the needs and preferences of patients in the delivery of high quality, high value care, cost effective care across the continuum. Our department is incorporated within the structure of the Department of Nursing and follows the mission, vision and governance structure set forth by the executive chief nursing officer.
Your Role :
Monroe Carell Jr. Children's Hospital at Vanderbilt is seeking a Manager, Patient Care Services for the Care Coordination Department . The Manager, Patient Care Services is a leadership role that has comprehensive operational authority and responsibility for designated clinical area(s). Their responsibilities include: fiscal accountability, maintaining quality and regulatory standards, maintaining a high level of patient/family centered care and service, patient and staff satisfaction, program development, managing and developing teams, and ensuring integration with the clinical enterprise. Provides feedback, coaching and mentoring to less experienced peers and other members of the health care team.
ESSENTIAL FUNCTIONS:
- S upervise s and coordinate s training, operations, and other activities for case manager staff .
- Supports case managers on a daily basis, keeping abreast of difficult/complex cases, and coaching, guiding, and assisting as necessary.
- P rovide s analysis and feedback on process improvement activities, and develops and presents focused educational programs for staff and physicians, both individually and in group settings.
- C ollaborat es in the ongoing re-design and re-engineering of Care Coordination functions , to meet the changing needs and priorities of the organization
- Evaluate s case management staff performance, c oach es and continually assess es learning needs of Case Managers .
- Educate s and serves as a resource for the use of software programs that support care coordination processes.
- Maintain s confidentiality and protects sensitive data at all times, including patient information, proprietary information and personnel information.
- Performs related duties as assigned.
Preferred:
- Master's Degree in healthcare related field
- Project management skills
- Previous pediatric leadership, case management and process improvement background
- Nursing certification would be a plus
