Discharge Planner Associate

Vanderbilt University Medical Center, Transitions Management Office (TMO) is accepting applications for the Discharge Planning Associate. This is a day shift with the capability to work 2-3 days per week to cover business needs and employee time off. Finally, you will have the opportunity to work with top-notch healthcare professionals.
In this role, you will facilitate and organize safe and timely discharge of patients requiring care coordination and resources. Advocates for patient/ family to referral sources and demonstrates a strong internal and external customer focus.
Additionally, you will typically receive information from Case Managers who make referrals of patients to home care, SNFs, Rehab facilities (Vanderbilt and non-Vanderbilt facilities). You may process between 15-20 patient discharges per day and may work on a multitude of units depending on the business requirement. Note, this position has limited direct patient interaction; rather you will be verifying criteria for patient placement, confirm insurance coverage, and facilitate service denials by insurance companies.

Hours: 8-5:30 pm (weekends may be required), position located within Vanderbilt University Hospital and Medical Center North. Finally, you will have the opportunity to work with top-notch healthcare professionals.
Position Qualifications:
Preferred:
- Technical School or the equivalent
- 3 years of healthcare experience or the equivalent
- Attention to detail and well-organized
- Ability to think critically and to work independently
