Licensed Clinical Social Worker – Integrated Substance Use Disorder Treatment Program $82,680 - $105,168 annual salary Do you have a keen professional interest in addiction medicine and substance use treatment? California Correctional Health Care Services (CCHCS) is seeking California-licensed clinical social workers to join our Integrated Substance Use Disorder Treatment Program. This newly-created statewide program is designed to provide integrated, specialized care to patients with a history of substance use who require additional medical assistance. Within this program and under the general direction of the Supervising Psychiatric Social Worker I, you will: - Organize and provide clinical social work services for patients with chronic diseases, including, but not limited to Substance Use Disorder, Hepatitis C, and HIV
- Serve as a an adjunct member of the Primary Care Team (PCT) and collaborate with other programs/units to screen, assess, recommend level of care, determine individual treatment needs, formulate treatment goals, and plan for disposition of patients
- Collaborate with internal and external partners to provide linkage of CDCR/CCHCS and community resources, facilitating patients’ effective adjustment into correctional settings and appropriate disposition from programs and/or community re-entry
