Job Title: Nurse Navigator, Outpatient Total Joints North Pointe Surgery Center (NPSC) is growing! Outpatient Total Joints is a brand-new program starting at our Lebanon NPSC facility. As such, we're adding several new positions including a full-time Outpatient Total Joints Nurse Navigator. The Nurse Navigator's key responsibilities include: direct patient care, education and comprehensive coordination of patient care, their families and caregivers; eliminate barriers to timely care; facilitate flow-through system; increase patient and provider satisfaction; point of contact with providers and serve as the first point of contact for patients and families. The ideal candidate must have the desire and ability to work a rotating schedule of day/evening hours, demonstrate friendly communication skills, strong organization and coordination skills, cultural openness, commitment to patient confidentiality, analysis/problem assessments, planning and decision-making skills and meet the qualifications of the position. Essential Functions - Promotes the mission and core values of the OAL Enterprise
- Adheres to the OAL Enterprise policies and procedures and may assist with educating new staff
- Ensures that prior to placing the patient into the Outpatient Total Joint Program that a Physician Peer Review has been completed on the case as outlined in the programs Physician Peer Review Policy.
- Oversees the planning, scheduling, and implementation of Outpatient Total Joint Program and procedures for navigated patients
- Ensures that the patient has an identified care giver that will be an active participant while the patient is part of the Outpatient Total Joint Program
- Educates patients and their identified caregiver about the patient's illness or injury, prescribed treatment, and side effects of treatment and how to manage them
- Conducts mandatory patient and caregiver education sessions no less than twice a month with at least one session in the evening.
- Assess patients for psychosocial distress and referral to a medical social worker or counselor if more intensive support is needed
- Answers questions as appropriate and serve as liaison to the physician for additional questions and concerns
- Addresses tangible barriers to care that may prevent a patient from participating in the Outpatient Total Joint Program.
- Communicates to physicians if barriers exist so that the patient may have their scheduled procedure performed in an acute setting.
- Ensures any health information provided is accessible: this may include securing documents printed in the patient's own language or arranging for certified medical interpretation, either in-person or by phone/video conference
- Functions as a member of an interdisciplinary team to assess, secure, and coordinate appropriate services through the continuum of care
- Supports the timely scheduling of appointments, diagnostic testing, and procedures
- Follows established entity policies, procedures, and objectives, continuous quality improvement objectives and safety, environmental, and/or infection control standards.
- Collaborates with medical providers, patient care staff and clinic management in the planning and implementation of patient and staff education
- May participate in the orientation, training and evaluation of new staff
- Designs or participates in the creation of health education materials
- Completes and reviews the necessary quarterly data to Pennsylvania Department of Health
