The Eastern Colorado Veterans Health Care System, Mental Health Service Line is currently recruiting for one (1) Social Worker Program Manager, Homeless Programs at the Rocky Mountain VA Medical Center, in Aurora, Colorado. You will be required to Operate a Government Vehicle. The CRRC is an off-site outreach program in Denver, Colorado serving Veterans experiencing homelessness. The CRRC Coordinator is delegated the managerial and supervisory responsibility for the CRRC as professionally appropriate. Has responsibility for coordinating interdisciplinary staffing, work assignments, budget, assessment, treatment, and referral services provided. Directly supervises a multi-disciplinary staff which includes outreach and assessment social workers, a community employment coordinator, a coordinated entry specialist and an HPACT social worker. Provides indirect or task supervision for peer support specialists, program support assistants, a psychiatrist, an HPACT nurse, and an occupational therapist. Coordinates with other Veteran service providers within the Veteran Administration and in the city, county, state and private sector to work as a cooperative team in the provision of service to homeless Veterans. Responsible for the fund control points assigned to the program, within the assigned areas. Responsible for the CARF accreditation activities, performance improvement and implementation of all directives related to the CRRC. Manages and controls the use of data related to this program regarding day to day functioning and program evaluation. Responsible for planning, organization and directing the administrative, operational, and patient activities for the CRRC. experience that demonstrates possession of advanced practice skills and judgment. At least two years of experience post-advanced licensure is in the specialized area of medical/psychiatric social work. Must be licensed or certified by a state at the advanced practice level. Directly supervises and/or coordinates a group of employees performing work at various GS levels (e.g. social workers, program support assistant, peer support specialists, etc.) and provides administrative, technical and clinical supervision necessary for accomplishing the work of the program including outreach efforts in the community and outreach efforts at other sites within the Health Care System. Expected to be heavily involved in community-wide efforts to end Veteran Homelessness. This includes active involvement with the local CoC and with community partners. Leader within the VA Homeless Programs in the creation and sustainment of a coordinated system to ensure homeless Veterans encounter "no wrong door" in receiving services. The CRRC Coordinator is the key lead between Outreach, Coordinated Entry, and homeless services. S/he must have advanced knowledge of homelessness and systems integration. Responsible for program needs such as material, equipment, and facilities needed to complete work. Administers information and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track service activities including visits, referrals, patient demographics, equipment utilization, housing, and patient satisfaction. Develops, implements, and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Responsible for submitting CRRC reports as required by the Medical Center, the VISN, and Central Office. These reports include NEPEC data, and other monthly and annual reports as required. Performs the administrative and personnel management function relative to staff supervised. Establishes guidelines and performance expectations for staff, which are clearly communicated through the formal employee performance management system. Observes workers' performance; demonstrates and conducts work performance critiques. Provides informal feedback and periodically evaluates employee performance. Skill in coordinating and implementing one or more specialty program(s), across the facility. Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget.
