Virtual RN/Case Manager II
The digital RN cares for patients virtually within the scope of the nursing practice license. Leveraging a variety of technology tools and software, the digital RN is responsible for the clinical monitoring and care for the patient including, but not limited to: clinical assessment, evaluation of clinical information that is being monitored and collected from patients, managing clinical protocols, assessing psychosocial needs, recommending services for the patient to the provider (i.e home health, DME, rehab services), creating collaborative care plans, performing case management functions and escalating clinical concerns to a provider in accordance with established protocols. ***Please note the hours of operation will be 7:00am -8:00pm, with a weekend rotation required
