Registered Nurse - Patient Access Coordinator
The Patient Access Coordinator is responsible for effective coordination, monitoring and facilitation of services necessary to meet the acutely hospitalized Veterans' medical and/or health needs in the community. The Patient Access Coordinator assists in care coordination for VA services, referrals and consults based on Veterans' eligibility status and services offered at VA Puget Sound Health Care System (VAPSHCS). The coordinator must also manage and facilitate transfers into VAPSHCS. As Nurse Coordinator, job duties, tasks and responsibilities include but are not limited to: Meets the needs of customers while supporting VA missions including ICARE values; consistently communicates and treats internal and external customers in a courteous, tactful, and respectful manner.
