Position Summary:
The Integrated Mental Health Therapist-BHN is responsible for independently providing assessments, treatment/care planning and psychosocial therapy within a primary care clinic setting for a diverse population of preventative, acute care and chronic disease management patients. The incumbent uses a variety of theoretical treatment models and interventions and excellent communication skills to work within a primary care/chronic disease management team setting with limited supervision; documents treatment/communication in keeping with department standards and keeps current on community resources to effectively provide linkage and advocacy to fulfill clients needs. Principal Accountabilities- Provides mental health evaluations, and based on results, utilizes the DSM-5 to provide an accurate diagnosis/differential diagnoses.- Provides person centered treatment planning based on the evaluation.- Employs various therapeutic interventions designed to resolve indentified issues and support needs.- Documents individual and group sessions and employs concurrent documentation as a tool to increase client engagement and ownership, whenever possible.- Documents communication with primary care clinic providers and outside support resources- Keeps current on community resources that may be needed to fulfill clients' needs and to provide linkage and advocacy and relays information, as needed.- Collaborates with other primary care team members to enhance the patient experience and facilitate positive health outcomes- Establishes and maintains productive relationships with all primary care team members and outside resources associated with the primary care clinic to facilitate quality patient care.- Meets agency documentation standards- Maintains agency productivity standards- Contributes to the success of the organization, keeping current on new developments with the MultiCare system, and by performing other duties as needed or assigned- Adheres to MHS Attendance and Punctuality Policy and Procedure standards, and maintains reliable attendance- Contributes to the success of the organization by meeting organizational competency expectations and core values (respect, integrity, stewardship, excellence, collaboration and kindness), continuously learning, and by performing other duties as needed or assigned
Minimum Requirements:
1-2 years of experience in a behavioral health setting
Education/Licenses/Certifications:
Masters degree from an accredited institution in Social Work, Counseling, Marriage & Family Therapy, or related field
Current licensure as a Licensed Clinical Social Worker Associate (LSWAIC), Licensed Advanced Social Worker Associate (LAASW), Licensed Mental Health
Counselor Associate (LMHCA), or Licensed Marriage and Family Therapist Associate (LMFTA) or eligibility for licensure upon hire.
