Location: This is a fieldwork position
The community you’ll be caring for: AdventHealth Hospice Care
The role you’ll contribute:
Performs responsibilities of coordination and supervision of hospice services to patients after hours and when the office is closed. Performs intake function, scheduling and clinical support for clinicians working. The RN provides skilled professional nursing care and support to patients and families after hours as ordered by physician and in compliance with regulations and standards. The RN provides care when needed as issues and/or emergencies arise. The RN follows the plan of care and assesses for needed services or changes in level of care. The RN assumes responsibility for implementing POC updates with collaboration from the physician on call or attending on case. The RN is responsible for accurate documentation for all visits. The RN may provide visits upon expirations.

Qualifications
The expertise and experiences you’ll need to succeed:
- Two years’ experience in hospice
- A clear/active Florida Registered Nurse (RN) License and Basic Life Support (BLS) certification is required. Certified Hospice and Palliative Care Nurse (CHPN) certification is preferred.
