Atlantic Health System is seeking a full-time Inpatient Care Manager for their Overlook Medical Center location.

• Coordinates patients through the acute care hospitalization
• manages all aspects of discharge planning/transition process
• performs utilization management and quality screenings

Qualifications:

Minimum Education:

• High school diploma or equivalent required

Education:

• 
  BSN preferred

License/Certification:

• 
  Valid New Jersey Nursing License required
• 
  CM certification preferred

Relevant Experience:

• 
  2+ of acute care experience strongly preferred
• 
  Case Management, Managed Care and /or Home Care experience preferred
