The Post 9/11 Miltary2VA Case Management Program (M2VA) Program Manager ensures that transitioning Service members (SM) and Post-9/11 era Veterans receive patient-centered, integrated Whole Health care, requiring a high degree of clinical oversight and creative problem solving. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Preferred Experience: Master's Degree in Nursing (MSN) 3-5 years nursing experience in one of the following areas: Medical/Surgical, Mental /Behavioral Health, Ambulatory Care. 1-2 years Supervisory or Management experience. Case Management Certification Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Jackson VAMC is a major interdisciplinary teaching facility, serving medical, surgical, and neurological inpatients and outpatients. Provides administrative oversight of the Post 9/11 M2VA program to include planning, establishing, and implementing policies and procedures; developing program goals and objectives; and monitoring, operating, and evaluating the overall effectiveness of the program. Coordinates comprehensive program elements and services for preventive mental and physical health evaluations for program participants. Promotes and coordinates evaluations for the M2VA Program anywhere within the facility and any outlying Community-based Outpatient Clinics (CBOCs).Prepares program reports for the VA Central Office, VISN POC, and Medical Center leadership. Monitors procedures for referrals between the VA medical center, Military Treatment Facility and/or community hospitals that promote appropriate timely inter-facility transfers. Responsible for management of the Military Treatment Facility referrals and other identified post 9/11 SM/Vs located in the catchment area. Ensures registration of appropriate Veterans for clinical, administrative, and outcome purposes. Collaborates with Enrollment and Eligibility staff to ensure access to care. Establishes performance and outcome standards for the facility M2VA Program that promote quality and efficiency of services in coordination with the overall goals of the medical center and those of the VISN and VA Central Office, as well as evaluate and document the program's effectiveness. Monitors policy and procedures to ensure compliance with Joint Commission (JC), Commission on Accreditation of Rehabilitation Facilities (CARF), and other accrediting bodies and regulatory standards as needed. Research community resources (local, state, and national) that are available to provide continuity of care and to enhance the quality of life of the Veteran. Disseminates information on resources throughout the VISN. Explores and analyzes community resources including family-friendly delivery systems. Identifies unmet needs of the post 9/11 population and works collaboratively with facility, VISN, and VA Central Office staff to develop and implement solutions. Develops a data-driven continuous quality improvement program with established goals and outcomes to evaluate and document the program's effectiveness. Develops and implements a plan for outreach to extend services to post 9/11 SM/Vs who may benefit from care, but who do not presently use VA for their health care needs. Ensures that the M2VA Program is interdisciplinary and integrated with the inpatient and outpatient medical programs. Establishes collaborative relationships with physicians, psychologists, nurses, social workers, and other disciplines to foster their involvement in the care of Veterans in the M2VA Program. Manages the daily operation of the facility M2VA Program and provides accountability for program effectiveness and modification of service patterns to enhance customer satisfaction. Identifies gaps in services and develops a plan, in consultation with M2VA staff and other departments as appropriate that will make available the best possible resources in meeting program needs. Supervise and manage human resources including recruitment, recommending employees for appointment, promotion, or reassignment, assigning work and measuring productivity and quality of performance of subordinates, performing chart reviews, developing performance standards and competency assessments, determining continuing education and training needs, resolving conflict within the team, and recommending appropriate disciplinary actions. The M2VA Program Manager may also function as a registered nurse member of the interdisciplinary M2VA treatment team.
