The ideal candidate is able to work in team-based environment, complete comprehensive assessments, and actively engage the patient and family members with dignity and respect.
Essential Functions
• Exercise autonomy in medical decision-making and in the assessment, diagnosis and initiation of certain medical processes and procedures within the NP scope of practice.
• Conduct in-home post hospital follow up visits.
• Develop and maintain a plan of care with the care team and family members
• Expected to become familiar with patient needs
• Management of severely critical patients
• Write referrals, renew medications, order diagnostics and manage results.
• Complete POLST (Physician Order for Life-Sustaining Treatment) for patients as necessary.
• Advanced directive and advanced life care planning activities are a priority in this position.
Position Details
• Work Schedule: 4/10s and open to working on Saturdays
• Ability to travel within the Las Vegas throughout the workday as assigned
• Avg Daily Patient Census: 6/day senior population and some younger patients who are medically disabled and on Medicare supplemental program.
• No Call
• Experience with treating patients with multiple co-morbidities
• Possess advance critical thinking skills
• Excellent communication skills, both verbal and written
Requirements
• Must have or be eligible to have a current and unrestricted NP or PA license
• Certificate of completion from an accredited Nurse Practitioner Program or Physician Assistant Program
• Current Nevada DEA certificate required prior to start date
• BLS certification
• At least two year experience practicing as an N.P. or P.A.
• Two years of experience in Primary Care or Adult/geriatrics
• Must have valid driver’s license and insurance
• Must be comfortable conducting home visits with high-risk patients

• Spanish-speaking

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.

Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.

Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).
