The Perinatal Case Manager position supports to improve the maternal and infant health outcomes of by providing comprehensive and coordinated prenatal and early postpartum support services that integrates behavioral healthcare within the OB outpatient setting. The perinatal social worker focuses on case management, psychosocial counseling and support, client psychoeducation and health promotion in an integrated care model to ensure that women have access to the services and information needed to have healthy pregnancies and healthy babies.

Your day.
As a Perinatal Social Work Case Manager you need to know how to:
- Counsel patients during pregnancy and up to one year postpartum about specific psychosocial needs of patients by identifying psychosocial disorders.
- Assist in the screening, assessment, planning, arranging, and implementing of psychosocial programs
- Delivers psycho-education about perinatal mood and anxiety disorders in a group or individual setting
- Perform short term counseling and crisis intervention as necessary
- Provides psychosocial health counseling and support, including psychosocial care-planning, goal-setting, monitoring, follow-up, and psychosocial care plan revision.
- Maintains clinical documentation in Epic.
- Responsible for the content of education and counseling programs for both individual patients or groups of patients and serves as liaison to community, individuals and groups as appropriate.
- Assist in the implementation of community and social programs, including Prenatal Plus and the ROSES program.
- Coordinate with clinical, health education, community health and other staff members in the provision of services to patients. Provide referrals for services needed.
- Coordinate with other organizational entities involved in psychosocial and mental health services.
- Provide clinical supervision of students in graduate education programs
- Provide in-service presentations to staff as needed
Your experience.
We hire people, not resumes. But we also expect excellence, which is why we require:
- Graduate from a masterâ™s program required in the field of social work, professional counseling, marriage and family therapy, or equivalent behavioral health specialty.
- Current Colorado LCSW, LSW, LPC, or equivalent licensure required.
- 1 year of clinical work experience in an inpatient or outpatient setting, including field placement, internship, etc.
Preferred:
- 1 year of clinical work experience in an inpatient or outpatient setting, including field placement, internship, etc.
- 1 year clinical work experience, experience in perinatal field.
