The Christiana Care Health System is currently recruiting a Weekend Incentive Program (WIP) RN Case Manager. As a WIP employee, you will commit to work 3 out of 4 weekends in a 4 week period. This position is responsible for covering many departments and working a 12 hour day/evening shift on Saturdays and Sundays from 7a-7:30p.
We are seeking an energetic, motivated RN that wants professional autonomy and understand the importance of care coordination. The Case Manager is a vital member of the interdisciplinary team that is responsible to create, implement, and monitor treatment plans to ensure safe, timely, and effective transitions throughout the care continuum and discharge planning.

EDUCATION AND EXPERIENCE REQUIREMENTS:
Active Clinical RN License required; BSN required.
Minimum of 1 year of clinical experience in acute care preferred.
Case Management or utilization management experience desirable.
Case Management Certification required within 18 months of hire.
