The community you’ll be caring for: AdventHealth Home Care
The role you’ll contribute:
Functions as the home health and hospice expert in the inpatient setting as an advocate for patients facilitating the transition to home care or hospice under the indirect supervision of the Home Health and Hospice Director, and working with the Hospital Case Management Director. Assist in the hospital discharge planning process by identifying patients who have medical needs following discharge that may be met with homecare or hospice, obtain physician orders and coordinate the transition to home health or hospice. Provide education regarding home health and hospice services.

- Participate in daily rounds to identify patients that may be appropriate for home health or hospice services. Meet with patients to discuss options and what to expect, assess if meets eligibility criteria, obtain physician orders, working with case management honors patient choice and follows established procedures, make arrangements for HME, infusion or other needs.
- Serves as a clinical resource, confers with health care providers, promote educational opportunities as they present, and attend meetings as assigned.
- Demonstrate critical thinking skills, analysis, and sound clinical judgment in the evaluation process of patient and the patient’s support systems to address the physical, emotional, psychosocial, and spiritual needs of the patients/families appropriate to the age of the population served: adolescent, adult, geriatric.
- Responds timely to referral sources and conducts a systematic assessment of the physical, psychosocial, and functional aspects of the patient and his/her family and their impact on the outcome potential, informing the patient and family about the home health/hospice philosophy, goals, and services. Determines patient and family problems, issues, and needs, and communicates these needs to the home health team.
QualificationsThe expertise and experiences you’ll need to succeed:
- Professional Registered Nurse currently licensed in the state of Florida
- Minimum 2 years nursing experience
- At least 1 year of home health/hospice experience
- Valid Florida driver’s license, safe driving record and reliable transportation
