Twin Cities Community Hospital
Twin Cities Community Hospital, a 122-bed acute care hospital, has been caring for the communities of Northern San Luis Obispo County since 1977. With a medical staff of more than 200 physicians and 400 caregivers and support staff, the hospital specializes in emergency care, obstetrics, orthopedics, digestive disorders, wound care and a broad array of medical, surgical and outpatient services. Twin Cities Community Hospital earned The Joint Commission�s Gold Seal of Approval® with distinction for its knee replacement program and was the first hospital in San Luis Obispo County to earn the LGBTQ Healthcare Equality �Leader� designation from the Human Rights Campaign Foundation. Position Summary
This individual�s responsibility will include, but not limited to the following activities: a) assist with adoptions, abuse and neglect cases, including making referrals to state and federal agencies b) participate in the weekly Complex Case Review meeting and the daily patient care conferences c) provide resources and make referrals to community agencies for ongoing counseling and or continued care d) maintain an updated list of providers for post-discharge care e) Collaborative communication with post-discharge providers f) Update Case Managers on variance to post-discharge plans g) monitor and provide reports to the DCM and hospital leadership as requested h) maintain knowledge of regulatory requirements and community resources i) and other duties as assigned.
Qualifications:EDUCATION / TRAINING / EXPERIENCE:
Master of Social Work Degree. Minimum 1 year experience in acute medical setting. Excellent verbal, written and computer skills, excellent organizational and communication skills, knowledge of internal policies and State/Federal regulations related to discharge planning and social services.
LICENSES / CERTIFICATIONS:
Valid LCSW license in Social Work in the State of California.
SKILLS / KNOWLEDGE / ABILITIES:
Has knowledge and skills sufficient to provide crisis intervention and short-term therapies to patients and families dealing with loss, grief, anger, depression, new environments, and changing physical capabilities. Recognizes team approach to discharge plan and utilizes team member�s input. Assesses needs of patient and communicates those as appropriate to plan of action. Understands impact of illness on the patient and family. Has medical awareness of patient�s illness and potential needs. Networks with others in the community to provide the best care for patients in and out of the hospital.
