Inova Health System has an exciting opportunity as a Hospice Coordinator. This position is full time Monday-Friday with no on-call, holidays, or weekends. This position will remotely cover four of our hospitals, Alexandria, Mount Vernon, Fair Oaks, and Loudoun. Travel and In-person presence will rotate weekly between the sites. What You Will Do: As a Hospice Care Coordinator, you will ensure that physicians, nursing staff, case management staff, and patients/families understand Hospice Care and the resources available to patients in and out of the hospital. To help achieve our mission, you will identify and coordinate Hospice referrals for all hospice services. Monitoring and reporting outcomes of referrals to Hospice is of vital importance.
Responsibilities:
- Identifies patients, collaboratively with Case Management, who are appropriate for Hospice referrals.
- Explains Hospice philosophy, levels of care, goals and services to patients/families, nursing staff and physicians to ensure understanding of principles of care.
- Assesses care options based on referrals and rounding with Case Management staff, nursing staff and physicians when appropriate. Obtains written referrals from the attending physician.
- Assists in the coordination of transfer of care to home or other facilities.
- Demonstrates knowledge of community resources to meet patient needs.
- Utilizes standards, guidelines and pathways for care delivery
- Maintains knowledge of evidence-based practice and maintains professional competence through personal educational advancement.
- Provides in-services to physicians, nursing staff and Case Management as appropriate. Assesses knowledge of staff and provides information on an individual basis when indicated.
- Identifies trends and develops/implements processes and plans for addressing these trends appropriately.
- Assists with compliance aspects related to hospice
- Collect appropriate outcome and referral data and report to leaders on a regular basis
Requirements:
Education:
Bachelor's Degree in Nursing - BSN RN
Master’s in social work – MSW
Experience:
Two years of public health/ community health, medical/ surgical, palliative or hospice nursing/ SW experience. Hospital based nursing/ SW experience.
License:
RN license in VA
LCSW
Certification:
BLS through the American Heart Association
Professional Certification preferred in Hospice or Palliative
