Representative Responsibilities- Supervision and support a team of Behavioral Health Care Coordination Specialists to provide community based care coordination utilizing high fidelity wrap around model and/or principals to serve youth and their caregivers who qualify for OhioRISE tier II- moderate or tier II-intensive care coordination support and are experiencing serious emotional disturbance (SED), trauma, cooccurring behavioral health disorders, and who are engaged with one or more child-serving systems (e.g., child welfare, intellectual and developmental disabilities, juvenile justice, education) experience. The supervisor will ensure that each care coordinator maintains compliance with all outreaches, documentation, and billing requirements and that all tasks are completed within specified timeframe.
Key Functions - Supervision and coaching of team of 8 Behavioral Health Care Coordination Specialists to provide care coordination using High Fidelity Wrap model and/or principals in serving OhioRISE eligible youth and their families
- Triage and assign referrals to care coordination team based upon acuity, capacity and caseload maximums
- Ensure that outreaches, assessments, safety plans, and all documentation is complete and compliant with timeliness requirements as set forth by ODM
- Assists team in development of child and family centered care plan (CFCCP) and Child Family Team (CFT).
- Reviews and approves each CFCCP prior to submission for approval by Aetna
- Attend CFT meetings facilitated by care coordinators to provide support, feedback, and coaching as necessary
- Provide oversite of 1915 waiver expense and monitor for compliance with approved fund usage and prior authorization requirements
- Monitor caseloads and ongoing progress towards goals
- Ensure care coordination team completion of all required education and training
- Identify education gaps and needs of care coordination team and individuals
- Regularly communicate barriers, concerns, and progress to Clinical Manager
- Provide performance feedback on an ongoing basis and for annual performance review
Program ManagementLeads projects and activities of the OhioRISE care coordination team. Monitors and responds to situations that affect daily program operations as they impact patients and families. Assists in the development, maintenance and oversight of program policies and protocols as they interface with other Departments. Participates as a member of the HealthVine Leadership team, providing ongoing input to planning, evaluating, and implementing programs. Takes the lead in ongoing assessment of current needs of target client population and development of programs/approaches that are responsive to these needs. Consults on program evaluation strategies and collection of data for reporting requirements and quality outcomes.
Intake ProcessCoordinates referral process to ensure caseloads are appropriate and outreaches are conducted within 2 days of receipt of referral. Assist in strategies to engage youth and families. Support development of care plans and child family teams that represent family choice, strengths, goals and that are culturally appropriate. Fosters and maintains relationships with external agencies, schools, community programs and organizations that are sources of resources, supports and service coordination. Counsel and advocate for families. Collaborate with all formal and informal supports and providers to insure coordination and continuity of care.
Team LeadershipReduces team communication conflicts by enhancing team collaboration and transparency. Participates in a zero tolerance for an unsupportive staff environment by intervening with other staff if observing unsupportive behaviors and/or contacting the Director for intervention. Resolves conflict and supports staff in their professional empowerment. Chooses opportunities to offer problem solving and/or supports positive health care team dynamics, exercising diplomacy, patience and tact. Works with teams to ensure program services are provided in a safe and effective manner. Fosters relationships within CCHMC and provides leadership in care of youth with complex physical conditions/disabilities and their families through guidance and supportive education. Establishes collaborative relationships with community and school-based personnel to support and coordinate patient care.
Community DevelopmentDevelops relationships with the community providers, agencies and physicians to ensure that families have knowledge about and access to services. Assists team to coordinate and plan for transitions between settings and levels of care coordination. Organization and communication of information about identified community services and resources for families.
Process and Quality ImprovementParticipates in ongoing review of data and processes to improve program outcomes. Monitors service areas, recommends approaches for improvement activities, assist in the design of new processes, review, and report performance over time, improves and redesigns processes based on the evaluation, and reassess continuously. Participates in planning and implementing best practice approaches to improve outcomes.
