In this role, you will assist patients through all phases of a womenâ™s life by coordinating a system-wide delivery of referral, education, well health support, and resource support.
Our Center for Women & Infants at Saint Joseph Hospital was designed exclusively for new moms and babies. Every inch of this special floor is dedicated to the care, comfort and well-being of new mothers and infants. With 18 expansive labor and delivery rooms with large jetted tubs, a dedicated elevator that goes directly to the floor and 42 private rooms that serve as a home away from home after childbirth, Saint Joseph Hospital is a great place for families. We also have a Level III NICU on-site.

Your day.
As a Womenâ™s Health Patient Navigator you need to know how to:
- Assess Women seeking support and facilitate coordination of care. Coordinate with providers in the community to be the liaison for needed referrals, coordinating well health experiences, education and support for the women in our community.
- Oversee the patientâ™s needs and progression through the support systems in the community, outpatient arena, hospital system, education and well health offering and support in the community.
- Work collaboratively with all providers, mental health teams, service lines in the hospital, education, lactation, PT and other disciplines as needed.
- Provide and coordinate education to the interdisciplinary team regarding, including but not limited to nursing, radiology, rehabilitation therapies, pharmacy, dietary and case management.
- Develop teaching programs and provide education to patient and families based on their specific needs. Promote the development of community awareness of supporting women.
Your experience.
We hire people, not resumes. But we also expect excellence, which is why we require:
- Bachelorâ™s degree.
- Current BLS certification endorsed by the American Heart Association.
- Minimum of three (3) years of previous Womenâ™s Health experience.
Preferred:
- Current Colorado RN license or compact license with the ability to obtain Colorado RN license.
- Health Navigator experience or certification and ability to obtain certification within two (2) years of hire.
