Social Worker in the Homeless Programs focusing on the SSVF SUD Expansion for Veterans experiencing or at-risk for homelessness with Substance Use/Co-Occuring Disorders. This position provides direct SUD case management, treatment, and intervention support to SSVF grantees to assist in bridging Veterans into formal treatment services. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Consultation for Non-VHA eligible Veterans can be provided to the SSVF grantee case-manager to support bridging Veterans into community care.\nb. Screening/Assessment - The Social Worker will provide biopsychosocial assessment with SUD emphasis screening to develop a treatment plan/care plan. The incumbent must be knowledgeable in ASAM criteria for treatment engagement, Harm Reduction, CBT-SUD, Contingency Management, and MET evidence-based practice treatments.\nc. Care Planning/Goal-Setting - Comprehensive treatment plans with the Veteran will address the factors that contributed to homelessness, including SUD and other co-occurring factors within a recovery model framework. Case Management and Counseling - The SSVF SUD Social Worker will focus on early intervention services with individual case-management, therapy, and group access supports to SSVF Grantees. Group engagement may include VA utilization in SUDRP services, community programs, and community-based groups. Services may be geared toward transitioning Veterans into formal treatment programs upon verbalize readiness and desire for SUD care. The incumbent will collaborate with VHI SUDRP programming through case-conferencing and seamless coordination into formal VA SUD treatment services where/when appropriate for SSVF Veterans eligible for VA Healthcare. The provider may serve as individual therapist in gaps of accessibility to individual therapy in VA SUD programming and as clinically appropriate.\ne. Crisis Intervention - The incumbent is experienced in making rapid assessments and developing crisis management plans for admission to mental health inpatient programs. The SSVF SUD Social Worker has access to multiple resource directories in addition to previously developed resources to meet the demands of a crisis.
