THE THERAPIST JOB FUNCTIONS INCLUDE:

1. Provides case management services which support a positive treatment outcome.
2. Serves as therapist in IOP and continued care groups, as well as provide individual therapy as needed for SUD and/or mental health disorders.
3. Organizes and makes educational presentations related to behavioral health and recovery.
4. Develop patient-centered treatment plans based on results from the screening tools and bio psychosocial assessment.
5. Collaborates with medical staff and treatment team as a whole. 
6. Consults with family members and referral sources in order to coordinate patient care.
7. Ensures adequate case management services are identified and addressed.
8. Responsible for maintaining electronic health records which include documentation of treatment activity and progress.
9. Ensures that patient records are accurate, comprehensive and meets regulatory standards.
10. Day hours 8:30-4:30p and Evening hours 12:30-8:30p available
11. Must be willing to work in person as needed

Required Qualifications:

• Certification or Licensed addiction counselor from the Board of Professional
• Counselors and Therapists (ADT, CAC-AD, CAC-I, CAC-II, CSC-AD, LMSW, LGADC, LGSW, LGPC, LCPC, LCSW-C, LPC)
• Maintains a professional and positive attitude and approach to patients, staff, and visitors
• Complies with State/Federal or Certification/Licensure training requirements

SPECIAL REQUIREMENTS: If disclosed that the staff is in recovery from alcohol and drug dependency a minimum of five years continuous sobriety is required.
