This position is responsible for case management, coordination and provision of social work services for the Mental Health Outpatient Clinic. The Social Worker is responsible for a broad range of skills and services including completing psychosocial history and assessments, advance directives, treatment planning, case management, individual, couples and group counseling as well as the coordination of discharge plans for veterans to ensure seamless transition of care.
