Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Must be licensed or certified by a state at the advanced practice level and must be able to provide supervision for licensure. ["This Social Worker position serves as a program coordinator and is administratively responsible for a clinical program providing treatment to patients in the Domiciliary Residential Rehabilitation Treatment Program (DRRTP). Incumbent is responsible for Domiciliary Care for Homeless Veterans (DCHV) program oversight, monitoring and administration. General Duties: Oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. Provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. Serves as a clinical social worker in the Mental Health Division, Rehabilitation Program, at the Clement J. Zablocki VA Medical Center, in Milwaukee, Wisconsin. Provides treatment services to patients experiencing a wide range of both chronic and acute mental health conditions that are impacted by the abuse of alcohol and/or drugs, including psychosocial problems impacting the discharge planning process. Serves as a case manager and provides a broad range of social work treatment modalities services for veterans with complex needs (mental health, substance dependence, medical, family dysfunction, homelessness, unemployment and/or lacking financial resources, legal, and lacking social supports). Responsible for meeting the complex needs of these veterans through thorough psychosocial assessment and evaluation; intervention, crisis management; treatment planning, individual, family, and/or group treatment; patient education; and/or information and referral. Patient Care: Performs clinical social work functions and specialized clinical social work functions as approved on the incumbent's clinical social work scope of practice application. Independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Independently conducts psychosocial assessments. Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team. Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. Independently identifies high-risk patients and provides case management services. I Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Establishes and maintains effective therapeutic relationships with veterans and their families. Independently work with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills. Provides consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to treatment plan. Administrative: Tracks and analyzes data to evaluate and enhance the quality of services provided. Participates on Medical Center, VISN and/or National councils, committees and workgroups as designated. Participates in the professional peer review process and professional/administrative continuing education activities.
