SUMMARY OF POSITION:
Community Paramedicine Clinicians will work with patients in and out of hospital environment providing clinical care to patients who have been discharged from the hospital. These clinicians will also work with the various service lines, including physicians, nurses, care coordinators and other members of the health system to with various transitional care needs and to mitigate preventable readmissions. These clinicians will also provide real time feedback to physician and nursing staff on clinical findings of patients who might need follow up care.
MINIMUM QUALIFICATION(S):
Minimum of five (5) years' verified work experience as a Paramedic in the Commonwealth of PA providing 911 services
Must maintain Paramedic certification without restrictions in the Commonwealth of PA
Completion of approved Emergency Vehicle Operations Training program required within two (2) months of date of hire
PA DOH EMSVO eligibility required within two (2) months of date of hire
PREFERRED QUALIFICATION(S):
Bachelor's degree strongly preferred, but supplemental work experience may be substituted for the desired education requirements
