Position SummaryThe goal of care coordination is to assist in managing care, cost, and outcomes across the continuum of care. The Manager of Population Health supports the Clinically Integrated Networks Disease Management, Utilization Management, Case Management, and Population Health initiatives. This position works collaboratively with Clinical Integration in developing and implementing an electronic health information system. Additionally, this position works with data analysts to provide the care coordination team high level technical and analytic support. This includes, but is not limited to, identifying high cost/high risk members and performance analysis, which will guide the care coordination team.
