Case Manager II
Summary: The Case Manager II provides utilization management on all admitted patients; facilitates discharge planning, assists in coordination of patient transfers to acute hospitals and psych facilities; provides supervision for Case Management Assistants. Responsibilities: Completes discharge planning, meds/orders for assigned patients. Completes documentation in patient's EMR in timely manner. Completes biopsychosocial assessments within 48 hours on all assigned patients. Completes referral forms/applications/logs within Case Management department. Provides training for newly hired RN Case Managers within the Case Management department. Assists with process improvement projects. Provides supervision to Case Management Assistants.
