The LPN Clinical Care Coordinator is responsible for performing a wide variety of patient care activities to support primary care and specialty teams. The LPN Clinical Care Coordinator will work within the scope of licensure to support direct patient care through managing daily patient flow, complex care coordination, and supporting patient, provider, and team needs. Clinical skills and a team-oriented, positive, and strong work ethic are a must.
Primary Responsibilities:
- Collaborates with providers to answer patient questions, schedules same day patients, works with acute patient issues, and supports utilization and patient access
- Works with providers on assisting with complex patients - paperwork, phone calls, specialty appointments, insurance issues, etc.
- Manages daily patient flow and team preparation - anticipates the needs of the team based on provider’s schedules and staff coverage
- Collaborates by using a team approach with the primary care providers, specialty providers, and other members of the healthcare team to ensure coordination of services
- Coordinates patient care plan with extended care team members such as case management, behavioral health, pharmacy services, etc.
- Desktop management - assists team members with time consuming, complex issues, referrals, paperwork, etc.
- Prioritizes work that is time-consuming and time sensitive such as stat referrals and urgent patient needs
- Manages patient paperwork through partnership with the front office, medical records, and indexing
- Supports electronic patient portal message in the EHR
- Rooms patients if necessary to cover staffing shortages, breaks and lunches
- Engages in team huddles and meetings on a daily or weekly basis
- Engages in constructive conversations on how to improve processes and job functions to better serve patients

Required Qualifications:
- Graduation from an accredited school of nursing
- Current LPN license with Oregon State Board of Nursing
- Basic Life Support for Healthcare providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross)
- 3+ years of experience working as an LVN/LPN
- Access to reliable transportation and a valid driver’s license
- Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance. Preferred Qualifications:
- I.V. therapy and phlebotomy certification
- 5+ years of experience working as an LVN/LPN
- Experience with care coordination processes. Experience in managing complex patient care functions through a collaboration with providers
- Proficiency with electronic health records, managing electronic schedules, and engaging in telephonic outreach
