JOB SUMMARY This position is for Baylor Scott & White Health Plan (BSWHP)
The Behavioral Health CM (BH CM) provides BSWHP members and their families who may be experiencing or are diagnosed with behavioral health symptoms and/or disorders, i.e. mental health and substance use. The BH CM acts as a point of coordination for the member between their outpatient providers and support system in order to facilitate healthcare needs and address barriers that impact BH recovery.
ESSENTIAL FUNCTIONS OF THE ROLE
Familiarity with and the ability to navigate the complexities of MCOs.
Conducts comprehensive assessments and targeted screenings to determine the unique socio-cultural, behavioral health, and medical needs of the member.
Maintains thorough and timely documentation, including case notes, professional reports, letters, and case summaries.
Employs advanced practice methods in the development, implementation and evaluation of treatment plans.
Provides or supervises plan of care interventions to include resource and referral assistance, crisis intervention, prevention, education, patient advocacy, bereavement, therapy, and counseling as appropriate. Collaborates with the healthcare team and involves the patient and/or family in the development and implementation of plans.
Serves as a liaison with community resources and their staff to facilitate smooth transition and placement of the patient within the assigned service line.
Provides consultation and guidance to other case management staff. Collaborates with other health system team members to facilitate the interdisciplinary care of patients and achievement of positive outcomes.
Provides backup supervisory support for the department under the direction of department leadership. Leads performance improvement initiatives either within or out of the department. Leads or participates in clinical or scholarly research activities leading toward academic publication, presentation in regional or national conferences and/or certified educational opportunities.
Represents the hospital or the social work profession on internal and external community boards, professional organizations such as National Association of Social Workers or American Case Management Association, college or university councils, government or city initiatives, linking the health care system to the community.
Provides field instructor services to schools of social work for the purpose of training Bachelor or Master’s program interns to the hospital or health care setting.

- EXPERIENCE - 2 Years of Experience
- CERTIFICATION/LICENSE/REGISTRATION - Lic Clinical Social Worker (LCSW), Lic Masters Social Worker (LMSW), Lic Master Social Wrk AdvPrac (LMSW-AP): Must have one of the following through the state of TX:
- LCSW, LMSW, LMSW-AP.
- LCSW license, Managed Care Organization (MCO) and BH CM experience preferred
