The Registered Nurse - Community Nursing Home (CNH) Coordinator oversees the daily operation of the CNH program & Community-Adult Day Health Care (C-ADHC) program to develop policies and procedures, provide clinical oversight of the Veterans assigned to both programs, and to prepare reports for leadership. The position entails travel into the community to complete bimonthly face-to-face oversight visits to Veterans in Community Nursing Homes and Community-Adult Day Health Care facilities. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
