The Senior Social Worker practices in the Health Care for Homeless Veterans (HCHV) Program and serves a patient population with highly complex health and mental health problems. The incumbent serves as a Social Worker for the Contract Recuperative Care Program as a liaison and care coordinator for community based, residential treatment programs for homeless Veterans. Additionally, the Senior Social Worker will practice part time as a Grant and Per Diem (GPO) Liaison.
