Summary:The Case Manager II provides utilization management on all admitted patients; facilitates discharge planning, assists in coordination of patient transfers to acute hospitals and psych facilities; provides supervision for Case Management Assistants.Responsibilities:Completes discharge planning, meds/orders for assigned patients. Completes documentation in patient's EMR in timely manner. Completes biopsychosocial assessments within 48 hours on all assigned patients.
