The Surgical Service Referral Coordination Case Manager's dedicated responsibility is identifying community care eligibility and working with Veterans to determine the best modality to receive the care for those patients referred for specialty surgical care. The incumbent is knowledgeable of care coordination agreements and provides a patient-centered service focusing on assisting the Veteran in making an informed decision. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
