Senior Social Worker (GPD Liaison) The successful candidate in this position functions as the Health Care System's Liaison/Case Manager for VA Central California's Grant & Per Diem (G&PD) Program. Responsible for monitoring and coordination of services provided by community-based programs funded under VA's Homeless Providers Grant and Per Diem Program. The liaison is the key point of contact between the programs in the field and the National Grant & Per Diem (G&PD) Program Office. The success of the individual programs depends greatly on the rapport and collaborative efforts established between the liaison and the community provider.
