JOB SUMMARYThis position is for Baylor Scott & White Health Plan (BSWHP)The Behavioral Health CM (BH CM) provides BSWHP members and their families who may be experiencing or are diagnosed with behavioral health symptoms and/or disorders, i.e. mental health and substance use. The BH CM acts as a point of coordination for the member between their outpatient providers and support system in order to facilitate healthcare needs and address barriers that impact BH recovery.  ESSENTIAL FUNCTIONS
