Registered Nurse
Summary: The Registered Nurse is responsible to accept and complete Home Care referrals received. This position assists in the development of appropriate Home Care plans with the referring institution, physician, patient/family/caregiver and Home Care staff. Additionally they work with established referral sources and develop new sources for home care referrals as well as increase the awareness of the scope of services that home care can provide for physicians and other health care personnel.
