About the Role:
Job Summary
The Orthopedic Nurse Navigator (ONN), a Registered Nurse, is responsible for the navigation of the patient through the continuum of health during the Joints in Motion 90- day episode of care. The ONN is responsible for the patient's coordination of care from diagnosis, pre-admission, intra, and post-discharge care. Significant implementation includes planning, implementation, coordination, monitoring, and evaluation of individualized care options, services, and resourcing.
Minimum Job Qualifications
Licensure or other certifications: Current license holder of the GA Board of Nursing, have or attain Orthopedic Certification (ONC) within six (6) months of hire, and belong to a National Association of Orthopedic Nurses (NAON) to stay abreast of current evidence-based practice
Educational Requirements: Bachelors Degree
Minimum Experience: Must have a minimum of two (2) years of direct patient care of orthopedic patients as well as providing education to the patient and family
Other: Participates in continuing educational activities as it related to the role and to maintain certification in area of specialty

Strong nursing skills with extensive orthopedic nursing knowledge and ability to safely triage patient concerns and make appropriate recommendations or escalate appropriately.
Strong communication skills, interpersonal skills, collaborative nature, and a positive presence.
Strong teaching skills and an understanding of group dynamics.
Analytical ability necessary to plan and coordinate the total joint program to ensure its effectiveness.
Essential Tasks and Responsibilities
Collaboration and professional communication with the orthopedic team to provide seamless care to the patient during the episode of care.
Educates and collaborates with orthopedic staff, Physicians, and the community as well as patients and their caregivers regarding services.
Using Evidenced Based Practice, implements chronic care management plan to assure optimal quality of life outcomes.
Assists patient/family in understanding their diagnosis, treatment options and resources available.
Assists with the education of staff members utilizing best practice standards, order sets in consensus with national guidelines.
Functions as an expert Clinician and mentor for staff.
Assists in tracking data, outcomes, and supporting strategic and performance improvement activities.
Practices LEAN principles to streamline care across the continuum, being mindful of cost and quality outcomes.
Leads education groups and monitors outcomes, e.g., patient satisfaction, compliance with treatment regimen.
Must have critical thinking skills, be able to problem solve independently, work well in a team environment. Strong written and verbal communication skills.
