Occ Summary

The Transplant Coordinator coordinates solid o rgan (heart, lung, heart-lung, liver, kidney, pancreas, and multi-organ) transplantation of allpediatric and adult organ recipients. The Transpl antCoordinatorprovides follow-up and care across the continuum from pre- transplant,during transplant and post transplant. S/he collaborates wit hmembersof the health care team to enhance patient care by developing,su pporting and educating nursing staff. S/he is a clinician, educator,inve stigator, consultant and leader. S/he makes clinical judgments basedupon a synthesis of current research, concepts, principles, theories,and outc omes to solve complex problems. The Transplant Coordinator is anintegral member of a larger interdisciplinary team contributing toexcellence inpa tient care, research, teaching, and in providingleadership to the organi zation.

Work Performed

Clinical Practice1. Provides advanced nursing care for patients before and aftersolid-organ transplan tation including heart, kidney, kidney-pancreas,liver and lung along the continuum of outpatient to inpatient 24 hours aday, 7 days a week.2.Coo rdinates evaluation process for pre-transplant (referral)patients toensu re effective use of hospital and clinical resources.3. Assesses post-tr ansplant organ function and general health statusand collaboratively det ermines need for additionalassessment orintervention.4. Facilitates the scheduling of studies or procedures fordiagnostic purposes utilizing fiscal responsibility for patient andhospital costs.5. Participatesin the development and implementation of protocoland standards of carefor the transplant patient in collaboration withthe multidisciplinary team.6. A ssists nurses and other staff to assess, plan, implement andevaluate the care of the transplant patient.7. Monitors the process and outcomes of routine care to determinequality improvement need.8. Provideson-call c overage, including coordination of activitiesprior to transplant, and as medical triage for out patients incollaboration with the physiilable (2 4hours/7 days a week) including communication between physiciancian on-c all.9. Coordinates transplantation process when a donor is avas, localo rgan procurement organization, patient/families andthe medicalteaminvolv ed including ED, OR, ICU (SICU, ACU), blood bank andhistocompatilbility labs.10. Completes necessary documentation according to theUnited Netwo rkof Organ Sharing (UNOS) guidelines including listing, status changes a ndpost-transplant follow-up information.11. Communicates with multidisc iplinary team members and patients andfamilies in a timely manner in reg ard to patient care/issues.12. Collaborates with PRM and other team mem bers in coordinatingdischarge plans forpatients pre-transplant or post- transplant includingreferral to home health / IV therapy / Hospice / Reh ab / SNF.13. Communicates with insurer case manager on an as needed bas is.14. Refers patients or families for assistance utilizinghealthsystem and community resources as appropriate.15. Maintains patient transplan t record with current data tofacilitate communication.16.Plans nursing c are and medical procedures with fiscalresponsibility by working collabor atively with transplantadministrators, financial coordinators, DUH utili zation review andthird-part case managers.Teacher/Educator1. Educates p atients along the continuum to assist understanding ofprocesses and pro cedures surrounding pre- and post-transplant issues.2. Develops patient education materials or identifies externalresources thatare appropriate for patient education in all phases oftransplantation care.3. Educates nurses and other multidisciplinary team members aboutpatient care issue s to promote positive outcomes for transplantpatients.4.Provides educati on and consultation about transplantation, andorgan donations for the Du ke Health System, public organizations orreferral centers/practices.5. Serves as a resource person/preceptor in organ transplant forgraduate an d undergraduate nursing programs.6. Participates or leads team conferen ces, rounds, or meetings inrelation to care of the transplant patient or program issues.Research Utilization and Scholarly Inquiry1. Interprets research and published data to nursing staff andcolleagues concerning c urrent issues in transplantation including thoseof a clinical, legal, et hical and economic nature.2. Incorporates research relating to the care of transplant patientsinto evidence-based practice while collaborativel y determiningappropriateness with the medical team.3.Identifies areas fo r study and participates in and/or designs andconducts research.4. Knowledge, Skills and Abilities

Attention to detail a nd effective communication and documentationwithnumerous physicians, org an procurement agencies, Operating Room (O.R.),Intensive Care Unit (ICU) and Emergency Department (ED) staff, thepatient and the patient's famil y are required.

Minimum Qualifications

Education

BSN required, MSN preferred.

Experience

5 years nursing experience with at least two years in transplant, critical care, disease specific OR minimum of 3 years clinical nursing experience in transplant, critical care, disease specific

Degrees, Licensures, Certifications

Current or compact RN licensure in the state of North Carolina required. BLS required.
