The Lead HUD-VASH Case Manager functions as Eastern Kansas Health Care System's (EKCHS) Lead for Housing and Urban Development-VA Supportive Housing (HUD-VASH) Program. The HUD-VASH Program which is located at both the Topeka and Leavenworth campuses is a collaborative effort, supported through HUD Section 8 rental assistance vouchers and VA's provision of intensive case management services. This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until December 29, 2023. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensed or certified by a state to independently practice social work at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. Senior Social Worker, GS-12 Experience: At least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment and are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. The primary component of the program is VA case management services, designed to improve veteran's health and mental health, enhancing their ability to remain stable, housed and community-integrated. The Lead HUD-VASH Case Manager performs social work duties as outlined in Social Work Professional Practice, VHA Handbook 1110.02 (6, 3 a-1). These social work functions are specifically directed toward homeless veterans and their families or veterans at-risk for homelessness and require a senior level clinical social worker. The Lead HUD-VASH Case Manager is also responsible for program oversight, monitoring and administration, serving as the primary contact with local Public Housing Agencies (PHAs) and providing and overseeing case management-based services (directly or through referral) necessary to ensure that veteran care is coordinated with the VA medical center, community health or mental health providers, and agencies that serve homeless populations. Due to the community housing focus of the HUD-VASH program, the work is to be done where the veteran resides, therefore, 70% or more of the HUD-VASH case manager time will be spent at a community location other than the VAMC or CBOC. The lead will not only promote effectiveness in the incumbent's social work function but oversee the effectiveness of other HUD-VASH staff. Veterans eligible for HUD-VASH come from a wide geographic area. Their health and mental health needs may be complex and require a high degree of clinical oversight and creative problem solving. Services are provided in healthcare settings and in the community. The HUD-VASH Program requires continuous coordination with the community's homeless providers and coalition as well as with other local, state, and federal agencies. Duties/Responsibilities include, but are not limited too: Conducts psychosocial assessments and screenings to determine Veteran eligibility for the HUD VASH program. Participates in the treatment planning process with other disciplines and the HUDVASH team. Acts as liaison between VA and community resources, to include marketing and public relation duties as needed. Provides case management to veterans and families, as needed, throughout the continuum of care. Develops working relationships and agreements with other organizations and directly coordinates with the designated PHA regarding housing voucher application and award process for each veteran. Reviews agreements between Section 8 landlords, PHS's and the VA to establish appropriate referral sites for placement. Has accountability for full utilization of the vouchers in his/her catchment areas and oversees the determination of when veterans are eligible for HUDVASH services as well as when they no longer need case management. Coaches the HUDVASH team in problem solving, provides advice on work methods, practice, and procedures, and assists the HUDVASH team and/or individual members in identifying the parameters of a viable solution. Leads the HUDVASH team in identifying and distributing and balancing workload and tasks among employees in accordance with established workflow, skill level and/or occupational specialization, making adjustments to assure timely service provision and to accomplish the program mission. Provides clinical supervision to social worker student or a less experienced social worker. Coordinates treatment and communicate with the care lines to facilitate continuity of care for veterans. Maintains data and statistical compilations to comply with JCAHO, CARF, VA and medical center policy and procedures. Maintains familiarity and responsibility for all performance measures relating to the HUD-VASH Program.
