OverviewThe St. Luke's Health System Hospice team is looking for a part time 0.8 RN Care Coordinator/Case Manager. Hours: Between Monday - Friday (8:30am - 5:00pm) with rotating evening call (5:00 pm-8:30am). The care provided by our Hospice team allows people to pass with dignity in their own homes, in familiar surroundings, with those they love by their side. The RN Care Coordinator works with physicians and multidisciplinary team members to develop and manage a plan of care for an assigned case load. This is a direct care position providing in home care that requires traveling within the Treasure Valley area. One of our Hospice RNs commented: "I love my job!
