Overview This role is a community health worker supporting the quality team and nurses in prevention and wellness visits. A trusted member of the population health team, the work may encompass basic clinical assessments based on standard criteria or protocols, data and chart abstraction, gap closure, outreach and scheduling services and referrals for individuals, diagnostic services, and preventive/annual wellness visits.
