Position Highlights
$15,000 sign-on bonus
Maintain appropriate credentialing and certifications as an Advanced Practice Clinician
Complete Member Health Screenings on Medicare Advantage Members in various clinical settings. These settings include but are not limited to; Skilled Nursing Facilities under contract to admit Optum Members, and the Optum Mobile Clinic at its designate location, and contracted primary care clinics. The primary execution of these visits is face to face in a clinical setting. However, Member Screening visits may also be accomplished in Member’s homes, or virtually through an approved telehealth medium
Assist with patient education, internal process improvements, and strive to improve patient satisfaction
Conduct peer reviews to ensure high quality documentation
Participate in Optum sponsored events (i.e flu shots events, open house events, marketing events)
This position involves care at the Mobile Unit, and Skilled Nursing Facilities within a specific geographical area. You must be comfortable working autonomously

Required Qualifications:
Idaho Nurse Practitioner or Physician Assistant license and national certification (Family Medicine, Adult Medicine, or Geriatrics)
Active DEA license with prescriptive authority, or ability to obtain
Proficient in assessment and patient education
Idaho Driver’s License and Auto Insurance in accordance with the state of Idaho (or will be licensed and insured by employed date)
Proven ability to work autonomously
Preferred Qualifications:
2+ years of advanced practice clinical experience working as a Nurse Practitioner or Physician Assistant
Experience in Gerontology, or Cardiology, or Internal Medicine, or Endocrinology
Knowledge and or experience working with the Medicare Advantage Population performing patient assessments/annual wellness exams
Knowledge of value-based model medicine including HCCs and Medicare HEDIS/Stars measures
