OverviewThe Case Management Representative supports the implementation of care management services to select patients in the community. This position will assist patients and caregivers in navigating community resources to reduce barriers to care and improve health outcomes. As a Case Management Representative, this position will perform outreach and intake interviews to collect patient-level information. Works collaboratively with providers and clinical staff to ensure prompt coordination of care and follow up.
