OverviewThe Manager of Patient Case Management administers the day to day operations of the Social Workers and the Discharge Specialist in the Patient Case Management Department, works collaboratively with the Patient Case Management leadership and the Patient Case Management Physician Advisor to carry out department goals. Responsible for implementing and maintaining daily social work departmental operations with consistent standards of practice.
