The incumbent is assigned to the Wilmington VA and provides services to those treated in the Homeless Veteran Program. In the provision of care in patients from the adult to the geriatric, the incumbent recognizes the age related needs and demonstrates knowledge & care appropriate to that age group. The social worker provides outreach, assessments, referrals & case management services to homeless veterans identified in the community as a part of H-PACT (Homeless - Patient Alignment Care Team). Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
