OverviewThis position provides support to patients through education and coordination of resources, support programs, and services to assist with health related needs. The Case Management Coordinator guides patients and their families through the healthcare continuum by identifying barriers to care and assisting with access to needed resources to decrease barriers. This position performs follow-up tasks identified during the clinical/hospital assessment for management of patients across the healthcare continuum or when the patient is in the continuum and needs additional resource support.
